2025 IRMAA Brackets

The 2025 standard Part B premium is $185.00 per month. IRMAA adds a surcharge once your income passes a bracket. Your 2025 surcharge is based on your income from two years prior (2023). The first cliff is at $106,000 for single filers and $212,000 for joint filers. Below are the full Part B and Part D amounts for every filing status.

How the brackets work

IRMAA is a cliff, not a slope. Going one dollar over a bracket moves your whole surcharge up a step for the entire year. Because Medicare looks back two years, your 2023 income sets your 2025 premium. See how to stay under the next bracket →
