The 2027 standard Part B premium is $218.60 per month. IRMAA adds a surcharge once your income passes a bracket. Your 2027 surcharge is based on your income from two years prior (2025). The first cliff is at $111,000 for single filers and $222,000 for joint filers. Below are the full Part B and Part D amounts for every filing status.

IRMAA income thresholds are indexed to inflation, so next year's brackets can be estimated before CMS publishes them. The figures on this page apply an assumed inflation adjustment of about 2.25 percent to the current thresholds, following the cited projection source. The projected standard Part B premium is $218.60 per month, and each tier's Part B and Part D amounts scale from there.
Treat every number here as an estimate. CMS and SSA announce the official 2027 premiums and thresholds in roughly November 2026, and this page is updated the week they publish. If your planned income sits within a few thousand dollars of a projected threshold, leave a safety margin: the final threshold can move. For what the standard premium itself will cost, see the 2027 Part B premium; collecting Social Security already? The 2027 COLA net check calculator shows what your bracket does to your monthly deposit.
Medicare uses the most recent tax return on file, which runs two years behind. The MAGI on your 2025 return (filed in 2026) decides your 2027 bracket. That means your 2027 IRMAA is already largely locked in. If your income has dropped since 2025 because of retirement or another life event, you can ask Social Security to use newer figures with Form SSA-44. To lower a future year instead, see how to reduce your MAGI: income moves you make in 2026 affect your 2028 premium.
IRMAA is a cliff, not a slope. Going one dollar over a bracket moves your whole surcharge up a step for the entire year. Because Medicare looks back two years, your 2025 income sets your 2027 premium. Compare income before and after a planned move →
You may be able to reduce your surcharge by appealing with Form SSA-44.
