The standard 2027 Medicare Part B premium is projected at $218.60/month. Most people will pay the standard amount, but higher earners pay more, because an income surcharge called IRMAA is added on top of the standard premium.
Everyone with Medicare Part B starts from the same base: the standard monthly premium. If your modified adjusted gross income (MAGI) is above the first bracket, the Income-Related Monthly Adjustment Amount (IRMAA) is stacked on top of that standard premium. The higher your income, the higher the tier, and the larger the surcharge added each month.
Social Security uses the most recent tax return on file, which runs two years behind, so your 2025 income determines your 2027 premium. The brackets behave as cliffs: a single dollar over a threshold moves your entire premium up to the next tier for the whole year.

Each fall, CMS announces the standard Part B premium for the coming year and SSA publishes the matching IRMAA thresholds. The moment the official 2027 figures are released, every projected number on this page and on the 2027 IRMAA brackets page will be replaced with the final amounts. If you are planning 2025 income around a threshold in the meantime, leave a safety margin: projections can move by a small amount in either direction.
