2026 IRMAA Brackets

The 2026 standard Part B premium is $202.90 per month. IRMAA adds a surcharge once your income passes a bracket. Your 2026 surcharge is based on your income from two years prior (2024). The first cliff is at $109,000 for single filers and $218,000 for joint filers. Below are the full Part B and Part D amounts for every filing status.

How the brackets work

IRMAA is a cliff, not a slope. Going one dollar over a bracket moves your whole surcharge up a step for the entire year. Because Medicare looks back two years, your 2024 income sets your 2026 premium. See how to stay under the next bracket →
